Report the crime to law enforcement officials (police, naval investigative service, military police or Federal Bureau of Investigation) without undue delay. Obtain an application by downloading the document below, or by contacting the Crime Victim Compensation Commission (Commission) office at (808) 587-1143 or\u00a0<a href=\"mailto:cvcc@hawaii.rr.com\">cvcc@hawaii.rr.com<\/a>. Submit your application to the Commission within 18 months of the crime date. data-id=\"https:\/\/dcr.hawaii.gov\/cvcc\/wp-\ncontent\/uploads\/sites\/2\/2026\/03\/Compensation_Application_2026.pdf\">\nNEW Application for Compensation\n<\/a>\nand \n<a href=\"https:\/\/dcr.hawaii.gov\/cvcc\/wp-content\/uploads\/sites\/2\/2026\/03\/Compensation_Brochure_2026c.pdf\" data-type=\"link\" data-id=\"https:\/\/dcr.hawaii.gov\/cvcc\/wp-content\/uploads\/sites\/2\/2026\/03\/Compensation_Brochure_2026c.pdf\">\nFAQs Brochure\n<\/a>\n\n\n\nAs part of this rollout, the Commission is conducting a <strong>soft launch <\/strong>of the new application while providing training to victim service providers and partner agencies across all counties.<br><br>Important Notice:<br>-Both the current application and the new application will be accepted.<br>-Processing of claims will not be affected during this transition.<\/p><\/div><\/span><\/div>\n\n\n\n<p><\/p>\n\n\n\n<p>To apply for compensation, submit the following:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>The original signed <strong><a href=\"https:\/\/dcr.hawaii.gov\/cvcc\/wp-content\/uploads\/sites\/2\/2024\/01\/Application-Only-2024.pdf\" target=\"_blank\" rel=\"noopener\">Application Form<\/a><\/strong>.<\/li>\n\n\n\n<li>The original signed&nbsp;<strong><a href=\"https:\/\/dcr.hawaii.gov\/cvcc\/wp-content\/uploads\/sites\/2\/2022\/09\/Medical-Authorization-2023.pdf\" target=\"_blank\" rel=\"noopener\">Authorization to Release Medical\/Mental Health Treatment Information Form<\/a><\/strong> for each treatment provider.<\/li>\n\n\n\n<li>Proof to substantiate your claim (bills, receipts, insurance statements, and medical records).<\/li>\n\n\n\n<li>If you are making a claim for lost wages, completely fill out and sign the <strong><a href=\"https:\/\/dcr.hawaii.gov\/cvcc\/wp-content\/uploads\/sites\/2\/2022\/09\/Lost-Earnings-Authorization-2023.pdf\" target=\"_blank\" rel=\"noopener\">Authorization to Release Employment Information Form<\/a><\/strong> and submit it to your employer.<\/li>\n\n\n\n<li>Submit proof to substantiate your claim for lost wages (pay stubs, Income Tax returns if self-employed, and a medical (Vietnamese)<\/a> <\/strong> <\/p>\n\n\n\n<p><\/p>\n\n\n\n<p>How will my application be processed by the Commission? <\/p>\n\n\n\n<p>Once we have received your application, the Commission staff will subpoena the police report and determine if you are an eligible crime victim. You will receive a written administrative decision and order either awarding or denying compensation. <\/p>\n\n\n\n<p><\/p>\n\n\n\n<p>What if I disagree with the Administrative decision? <\/p>\n\n\n\n<p>You may appeal the Commission\u2019s decision in writing within 60 days of the mailing date of the administrative decision and order. The Commission will set a hearing for your appeal and you will have an opportunity to tell the Commission why you disagree with the decision and to present documentation. The Commission will make a decision and provide you with a written notification of the decision. <\/p>\n\n\n\n<p><\/p>\n\n\n\n<p>What if I disagree with the Commission\u2019s decision? <\/p>\n\n\n\n<p>The Commission\u2019s decision may be appealed to the Circuit Court of the victim\u2019s county of residence within 30 days of the mailing date of the Commission\u2019s decision and order.
